Designing for Dissemination and Sustainability (D4DS) Planner

This project developed a website (the D4DS planner) that supports researchers and practitioners in learning about and applying the methods and principles of designing for dissemination and sustainability.

Project goals:

  • Develop a website using a human-centered design approach that provides users with tools to develop an innovation (research discoveries/interventions/digital health tools) using D4DS processes that engage stakeholders to increase the potential for dissemination and sustainability—resulting in more effective translation of research to practice and policy.
  • Conduct usability testing, iterate, and refine the website.
  • Actively disseminate the website to reach researchers and practitioners.

PULSE DeveloPing a pragmatic gUide to impLementing social riSk assistancE

This project aims to support primary care and community health clinics design Assistance strategies that will address identified social needs that are yielding barriers for their patients to receive screening and preventive care (e.g., those lacking transportation may be unable to go to recommended colonoscopy appointments).

Project Goals:

This supplement is a collaborative effort across four ISC3 Centers: BRIDGE-C2, Harvard ISCCCE, University of Washington OPTICC, and Washington University-ISC3.

  • Goal 1: Develop a pragmatic, applied guidebook (the Guide) for clinics seeking to implement, expand, or optimize efforts to address patient-reported social risks using Assistance strategies.
  • Goal 2: Use rapid cycle testing to identify best practices for implementing the Guide.
  • Goal 3: Iterate and disseminate the refined Guide.
